New testing approach in HLA genotyping helps overcome barriers in effective clinical practice.

چکیده
BACKGROUND Severe and potentially fatal hypersensitivity reactions to drugs, particularly antiepileptics, are clinically unpredictable. Recent evidence has revealed a strong and specific association between the implicated drug, the type of adverse reaction, and the particular HLA genotype. An urgent need exists for rapid diagnosis of HLA status to guide drug prescription; however, traditional HLA genotyping has a long turnaround time, is expensive, and is available only in specialized centers. We tested the feasibility of the loop-mediated isothermal amplification (LAMP)-based approach to detect a specific HLA genotype. As an example, we used B*1502, an HLA allele strongly associated with carbamazepine-induced Stevens-Johnson syndrome/toxic epidermal necrolysis, and validated the assay's application as a simple, accurate, rapid, and low-cost blood test for use in both clinical and bedside settings. METHODS We evaluated B*1502 status with the new LAMP method and compared the results with those obtained by sequence-based typing (SBT) (n = 250) and by sequence-specific primer PCR (SSP-PCR) (n = 200) for 450 samples of DNA (n = 50) and blood (n = 400) from a hematology laboratory. RESULTS LAMP results showed 100% concordance with both SBT and SSP-PCR results, confirming that LAMP detection of a specific HLA genotype (B*1502 in this case) is an accurate method. All results were available within 1 h. CONCLUSIONS Our study confirmed that the new LAMP method for detecting a specific HLA genotype is simple, inexpensive, accurate, and rapid, and may be of help in overcoming barriers in effective clinical practice.

Association between Hepatitis B Virus Infection Outcome and HLA-A and DRB1 Genotyping in North Part of Iran
Background and Objective: The outcome of hepatitis B virus (HBV) infection may be influenced by host factors like Human Leukocyte Antigen (HLA). We have investigated HLA-A and DRB1 alleles in patients with persistent hepatitis B infection compared to subjects who had spontaneously recovered from HBV infection. To complete the findings of this study we performed another survey in certain HLA ...

Background & Aim: In the recent past, traditional medicines have gained increased awareness among the scientific community and general public owing to the intrinsic value of these systems. They are considered because of the drug resistance and side effects associated with allopathic medicines. Also, in a country like India, where there is availability of rich medicinal flora, h...
متن کاملActive Learning: An Approach for Reducing Theory-Practice Gap in Clinical Education
Introduction: The gap between theory and practice in clinical fields, including nursing, is one of the main problems that many solutions have been suggested to eliminate it. In this article, we have tried to investigate its solution through active learning. Methods: In this review article, searching articles published during 2000-2012 was done through library references, scientific databases. ...
متن کاملThe Pharmacogenomic HLA Biomarker Associated to Adverse Abacavir Reactions: Comparative Analysis of Different Genotyping Methods
Many pharmacogenomic biomarkers (PGBM) were identified and translated into clinical practice, affecting the usage of drugs via label updates. In this context, abacavir is one of the most brilliant examples of pharmacogenetic studies translated into clinical practice. Pharmacogenetic studies have revealed that abacavir HSRs are highly associated with the major histocompatibility complex class I....
متن کاملHLA-Ig-based artificial antigen-presenting cells: setting the terms of engagement.
Recent advances in molecular medicine have shown that soluble MHC-multimers can be valuable tools for both the stimulation of as well as the analysis of antigen-specific T cells in vitro. In this review, we describe the use of dimeric major histocompatibility complexes, HLA-Ig, to visualize antigen-specific T cells as well as their potential to stimulate immune responses as part of an artificia...
